The program includes a response form that families can completeβProject SAFER Response FormIf you would prefer, you can print form.These forms will be stored in a First Responder database in accordance with The Health Insurance Portability and Accountability Act of 1996 (HIPAA). This federal law protects sensitive patient health information from being disclosed without the patient's consent or knowledge. First Responders will be able to access this information to better assist people. The form may be completed online, mailed, or delivered to Clallam County Department of Health and Human Services Public Health.β

The response form allows families to share communication preferences and needs with first respondersβIndividuals, families, care providers, and community partners will collaborate to ensure inclusive and secure emergency interactions by supplying First Responders with details regarding approach preferences, communication needs, and strategies.β
